The McPin Foundation is conducting a survey into people’s experiences of mental health crisis care, and your response would be appreciated.

This survey is for anyone to complete who:

• Lives in England
• Has experience of a mental health problem or/and cares for someone with a mental health problem who has sought urgent help for a mental health crisis from a service or professional.

Confidentiality:

• All answers given will be treated in confidence
• You do not have to give any contact details

What the information will be used for:

• Helping us to build a picture of crisis care support in England alongside the crisis care concordat work
• Finding out how crisis care support is changing in different parts of the country
• Using lived experience to influence the shape of future crisis services

When is the information needed?

• Deadline for this survey is Monday 1 December 2014
